Handled respectfully
Feedback is handled respectfully, confidentially and as promptly as possible.
Your feedback matters. We welcome compliments, suggestions, concerns and complaints, because they help us understand what we are doing well and where we can improve.
Participants have the right to raise a concern or make a complaint without fear that their supports will be reduced, withdrawn or negatively affected because they spoke up. This is a right, not a favour.
Feedback is handled respectfully, confidentially and as promptly as possible.
You may choose to involve an advocate or support person at any point. You do not have to do this alone.
You can raise a concern with the NDIS Quality and Safeguards Commission directly, at any time, without telling us first.
Submitting this form opens your own email application with your answers filled in, ready for you to send. If you would rather not use a form, you can call us on +61 483 363 867 or email admin@clearmindservices.com.au directly — either is completely fine.
You have the right to provide feedback or make a complaint, to have an advocate or support person involved if you choose, to have your privacy and personal information protected, and to continue receiving respectful support throughout.
Whether you are looking for support for yourself, a family member or someone you work with, ClearMind Services is here to listen and understand what you need.
